EXPRESS ORDER FORM

To place an order, print our Express Order Form and fax it to 317-545-0683 or you can place your order over the phone by calling 317-591-7260. We accept MasterCard, Visa, and American Express. Other forms of payment can be accepted at time of delivery if a credit card is used to guarantee your order. Established corporate clients can request to be invoiced. Orders must be placed at least three (3) business days in advance. There is a minimum guest count of at least 25 persons.

*Tax and delivery fees are extra. There is an 9% sales tax on food and beverage and the delivery fee is $25.
